Terms & Conditions (Please note - T&Cs are a binding contract):

  • No HICAPS Machine Onsite: There is no HICAPS or Medicare claiming available onsite at Body Form. All clients are responsible for self-claiming their treatments after they have been completed. Body Form will provide invoice receipts for all payments for your records; however, we do not take any responsibility for submitting claims on your behalf. Clients must not upload or claim any treatments they did not physically attend. Claims are only valid for attended sessions. Body Form holds no responsibility regarding any aspect of the claiming process.

  • Physiotherapy Appointments: A Physiotherapy appointment is required every 12 weeks to continue claiming under Private Health.

  • Pre-booking and Prepayment: All sessions must be pre-booked and prepaid.

  • Cancellation Policy for Physiotherapy and Private Classes: A 24-hour cancellation policy applies. If cancellation or no-show occurs, the full fee will be charged (credit lost), and an additional $15 cancellation fee will be charged to the account.

  • Cancellation Policy for Group and Semi-Private Classes: A 12-hour cancellation policy applies. If cancellation or no-show occurs, the full fee will be charged (credit lost), and an additional $15 cancellation fee will be charged to the account.

  • Non-refundable Sessions: Sessions are non-refundable, non-transferable, and non-redeemable for cash.

  • Payment Schedule: Payments are debited weekly.

  • Session Duration: All sessions are 50 minutes in duration.

  • Membership Cancellation Notice: A 2-week notice is required to cancel memberships after the 12-week minimum period. You have until the end of the 2-week cancellation notice period to use any remaining class credits. After this period, unused credits will be voided and nonredeemable, and no extensions will be granted.

  • Unpaid Sessions: If a class or physiotherapy session is unpaid, we reserve the right to charge your card on file after 24 hours from the session's commencement.

  • Card Balance: Please ensure the required balance is available in the allocated card at all times.

  • Membership Pause Policy: One membership pause is allowed per 8-week block. You must notify via email of the requested pause date; otherwise, the debit will continue. There is a $15 membership pausing fee per week for all packs and memberships if you wish to extend.

  • Class Modifications: Classes may be canceled, changed, or modified at the studio’s discretion. Alternatives will be provided to fulfill membership requirements.

  • Attire Requirements: Please wear comfortable exercise clothing, and note that pilates socks are required.
